Checklists within a Culture of Human Performance

People make mistakes.  The study of human fallibility and human performance has uncovered a universal truth, that people make mistakes.  Human error is not 100% preventable, so organizations must build systems and processes that trap or minimize the effects of human error.  One solution that is gaining traction in high-reliability organizations is the utilization of checklists.  This presentation will look at how a checklists culture can be established and why checklists only drive intended consequences in the correct environment.  The simple truth is:

  • Even experts need checklists because, humans are fallible
  • Well-written operational checklists don’t teach you how to do the job – they mitigate human factors (e.g. stress, distraction, fatigue, etc.) and ensure critical items are accomplished
  • Checklists are only effective if they reside within a reinforcing culture
